Neurological Deterioration after Decompressive Suboccipital Craniectomy in a Patient with a Brainstem-compressing Thrombosed Giant Aneurysm of the Vertebral Artery

چکیده
We experienced a case of neurological deterioration after decompressive suboccipital craniectomy (DSC) in a patient with a brainstem-compressing thrombosed giant aneurysm of the vertebral artery (VA). A 60-year-old male harboring a thrombosed giant aneurysm (about 4 cm) of the right vertebral artery presented with quadriparesis. We treated the aneurysm by endovascular coil trapping of the right VA and expected the aneurysm to shrink slowly. After 7 days, however, he suffered aggravated symptoms as his aneurysm increased in size due to internal thrombosis. The medulla compression was aggravated, and so we performed DSC with C1 laminectomy. After the third post-operative day, unfortunately, his neurologic symptoms were more aggravated than in the pre-DSC state. Despite of conservative treatment, neurological symptoms did not improve, and microsurgical aneurysmectomy was performed for the medulla decompression. Unfortunately, the post-operative recovery was not as good as anticipated. DSC should not be used to release the brainstem when treating a brainstem-compressing thrombosed giant aneurysm of the VA.

Background Kawasaki disease (KD) also known as mucocutaneous lymph node syndrome is the febrile and self-limited vasculitis that occurs in children of all ages, especially younger than five years of age. Here, we describe a case of Kawasaki disease with coronary artery aneurysm and a big clot in LV with a diameter of 1.5 cm. <stron...
متن کاملGiant thrombosed vertebral artery aneurysm treated by extracranial-intracranial bypass and aneurysmectomy--case report.
A 42-year-old male presented with a giant thrombosed aneurysm of the left vertebral artery. The aneurysm was resected through a combined subtemporal and suboccipital approach after a saphenous vein bypass graft was placed between the right external carotid artery and the posterior cerebral artery. Intraoperative measurements of the blood flow volume and pressure demonstrated good blood flow in ...

Unilateral posterior inferior cerebellar artery (PICA) thrombosis is frequent. However, bilateral PICA thrombosis is rare. Herein we report about an intraoperative visualization of a bilateral thrombosis of the telovelomedullary segment of the PICA. A 74-year-old woman was admitted to our department on day two of a bilateral PICA thrombosis with developing cerebellar infarction. Her Glasgow Com...

BACKGROUND AND PURPOSE We present an anecdotal series of 11 patients without past history of stroke with progressive neurological deterioration while on medical therapy for large cerebellar infarctions. Clinical signs of brain stem compression developed in these patients. Computerized tomography of the head confirmed mass effect from brain edema. It was the clinical judgment of the neurologists...
متن کاملGrowth of a thrombosed giant vertebral artery aneurysm after parent artery occlusion.
Parent artery ligation is an accepted treatment for intracranial aneurysms that are not amenable to a direct surgical approach. Parent artery ligation is one of the oldest effective surgical means of aneurysm therapy (the "Hunterian principle" (1 ]). Recently it has become possible to perform parent artery occlusion of the carotid or vertebral artery percutaneously by using detachable balloon t...
